• 30 minutes telephone consultation - FREE

• 1 1/2 hour Occupational Therapy Assessment includes 2 copies of your assessment - £350 *

• DFG Occupational Therapy assessment and includes a copy of the assessment recommendations for adaptations, liaising with the local authority surveyor /technical officer to approve schedule of works. - £580 (mileage extra). This cost can be claimed back via the DFG application.

• Moving and Handling Assessment includes a copy of risk assessment and manual handling care plan assess- £450 *

• Single Handed Care Reviews includes 2 visits , demonstrating new equipment provided with carers, risk assessment and care plan - £500 *

• Supervision/Mentoring- minimum 1 hour- £35*

• Follow up visit - £50 per hour *

- 10 miles + 45per mile
